**POSITION SUMMARY
The Substance Abuse Care Facilitator serves as a coordinator and collaborator between Hospital Emergency Departments, inpatient treatment programs and other community based services to behavioral health and primary care services. The Substance Abuse Care Facilitator utilizes engagement strategies and provides brief case management services to facilitate full linkage to services. This person works as a liaison between the client’s behavioral health provider, primary care provider, and other providers involved in the clients care facilitating communication and coordinating appointments as needed. May also provide education to the patient/family about their health care needs and recommended wellness activities.
**EDUCATION AND EXPERIENCE/QUALIFICATIONS
Bachelor’s in Social Work or other human services related field plus at least 1 year of related experience including face to face interaction with clients is required.
Case management experience in a health care setting is preferred. Spanish speaking is preferred.
A valid Connecticut driver’s license is also required.
Exhibits strong interpersonal, critical thinking and problem-solving skills. Demonstrates proficiency in Microsoft Office applications, other software programs, and office equipment such as fax, copiers, and scanners.

**ESSENTIAL DUTIES AND RESPONSIBILITIES
Provides case management services for a caseload of 35 – 40 clients.
Facilitates engagement and retention of clients into the Wheeler Clinic Health and Wellness programs through coordination and collaboration with Hospital Emergency Room Departments and other community based services.
Work in collaboration with Hospital Emergency Departments and other community based providers involved to identify clients in need of referral and engagement interventions.
Work in collaboration with all providers involved in the clients care (including behavioral health providers, referral sources, medical providers) to facilitate attainment of identified treatment goals.
Provides continuity for the client between care settings.
Attends, or conducts health care meetings when necessary to facilitate the coordination of complex services and resources.
Provide behavioral health screenings, education, and brief interventions to assist linkage to integrated behavioral health services.
Document care coordination efforts and communicates as indicated.
Provide education on importance of identifying a primary care provider.
Assists clients in how to utilize the health care system appropriately.
Maintains a working knowledge of the resources available in the community and provides appropriate referrals.
Provides education of available resources to clients and/or their families.
Actively seeks to increase participation in treatment by promoting treatment plan and medication compliance for difficult to engage clients.
Establishes relationships with providers and staff of the Hospital Emergency Departments, outpatient clinics, and other community based resources
Utilizes electronic reports to support tracking of patient activities.
Maintains professionalism with clients, staff and colleagues.
Maintains competencies to meet expectations in all position-specific key performance indicators.
Pursues and utilizes available trainings and staff development opportunities, to enhance knowledge and growth with the agency.
Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
