JOB SUMMARY:
The Case Manager oversees the provision of services and supports to individuals in accordance with each individual’s identified outcomes as well as The Arc’s Mission Statement and Guiding Values. This includes developing each individual’s Habilitation/Treatment Plan, assuring all documentation of service provision and assuring that services meet the goals and objectives as outlined in each planning document. The Case Manager is accountable to the person receiving supports, their team and the agency.
EDUCATION AND EXPERIENCE:
Required: Two (2) years previous social services work experience with case management responsibilities, including one (1) year in the field of developmental disabilities. Valid Alaska driver license, clean driving record, proof of insurance, current TB skin test/health screening and the ability to pass a criminal background check.
Preferred: Bachelor’s Degree in Human Services with a desired concentration of study related to developmental disabilities/mental health and/or social work.
